Once you have filled out and returned your PBG Declaration Form, you can set up an account and order or prebook your COVID-19 vaccines online at Moderna Direct. Customers may also order Moderna’s COVID-19 vaccines through one of Moderna’s partnered distributors. Please click here to view Moderna’s list of distributors.
Whether you choose to order through Moderna Direct or through one of their partnered distributors, you will still need to fill out and return the Moderna PBG Declaration Form for the 2024 season. If you have any questions, please contact us.
