To qualify for these discounts, new and existing members must fill out and return the PBG declaration form to Amy Bullock at abullock@primarycarealliance.com. We recommend our members set up an account at Moderna Direct to prebook and order Moderna vaccines. Moderna Direct offers a 2% prompt pay discount if invoices are paid within 60 days. They also allow credit card payments without an additional fee.
